MB4MH and Trilogy Care

MB4MH has partnered with Trilogy Care, an approved Home Care Package provider, to ensure you have access to high-quality home care services via your Home Care Package. Trilogy Care can administer your care package while you have the flexibility to work with the service providers of your choice. 

Both organisations are focused on empowering you to maximise your care hours and wellbeing by giving you choice, control and independence over your care.

Trilogy Care’s model empowers you to have the flexibility to choose your service providers and gain more control over how your Home Care Package budget is spent. This means you can continue to work with MB4MH and have MB4MH support services funded under your Home Care Package budget – subject to your care plan and budget limits.

Working with MB4MH and Trilogy Care means you will spend less on Home Care Package management fees so you can spend more of your Home Care Package budget on the things that matter – care and services to help you stay at home longer.

Who is Trilogy Care

Trilogy Care supports older Australians to live well at home for longer, by helping them build the Home Care Package they want and need.

What is a Home Care Package?

A Home Care Package is a package of services provided by the Australian Government that supports older Australians to live safely and independently in their own home.

The program is designed to support you with daily activities your care recipients may need assistance with, such as cleaning, showering, or attending appointments.

All Home Care Packages recipients need an approved Home Care Package provider to administer the Government subsidy.

How can MB4MH help?

Working with Trilogy Care and MB4MH, you will spend less on Home Care Package management fees, allowing you to spend more of your Home Care Package budget on the things that matter – care and services to help you stay at home longer.

What is Self-Management?

Self-management is a way for you to manage the service providers you use and how your Home Care Package budget is spent. It gives you the flexibility to: 

• Design and plan your own care plan with a dedicated care partner 

• Choose the service providers you would like to work with 

• Gain more control over how your Home Care Package funds are spent 

• Increase your care hours with lower management fees

Why Trilogy Care?

Trilogy Care has first-hand experience supporting Australians to self-manage their Home Care Packages. They draw on this experience to empower you to continue to live an independent, healthy, and supported life in the home you love. Unlike traditional providers that charge upwards of 30-35%, Trilogy Care stands out with its low fee structure, charging only a 15% or 26% fee for its services. 

This transparent pricing model ensures that care recipients receive exceptional care without incurring excessive costs, allowing them to maximise the value of their Home Care Package, and ultimately receive the maximum number of hours from the services you receive from MB4MH

Our services are available for self-managed individuals under aged care and disability funding arrangements. All support is delivered by a qualified, police-checked practitioner, holding a current NDIS Yellow Card and Blue Card for your safety and peace of mind.
